Welcome. This Directory is intended for use by participants in the New Jersey State Health Benefits Program (SHBP), and their enrolled family members. It is designed to provide all members -- employees and covered dependents -- with a single source for identifying physicians and other health care professionals, suppliers, and facilities who are affiliated with one or more of the medical benefits plans available under the SHBP.

Essentially, the Directory consists of two parts. One part lists individuals who practice the healing arts. The other lists hospitals.

Please note: The information in this directory is subject to change. It is essential that you contact the health professional or supplier, or your health plan, to verify their continuing participation in the health plan, before seeking their services. Also, you must follow your plan's rules regarding changing primary care physicians, seeing specialists, etc. Failure to follow your health plan's rules will result in the denial of payment as in-network care for the services received. In order to avoid such events, you should call your plan's member services office to:

  • change your primary care physician,
  • confirm the authorization of a referral to a specialist,
  • verify a hospital admission has been approved.
